ARCSCHÖN Aesthetics Model Program – Instructions

1. Select Photoshoot/Event Date
   Choose the Model Program Application option when selecting photoshoot/event date of interest. Please keep in mind an appointment for a treatment session will be scheduled 1 or more weeks prior to photoshoot/event upon acceptance. The calendar invite sent after submission serves as a reminder of your proposed participation in a photoshoot/event.
   Note: This calendar invite does not confirm acceptance into the program.

2. Complete the Basic Intake Form
   You’ll be prompted to fill out a brief form with your contact information, service interest, and basic health information. This may come in a separate email or display immediately after selecting the date of interest. This form helps us evaluate your eligibility for the program.

3. Application Review and Next Steps
   Our team will review your submission and contact you with next steps. If selected, you’ll receive an acceptance email with instructions for scheduling treatments, completing a telehealth Good Faith Exam (GFE), providing required intake and consent forms, and signing a separate Model Agreement.
